SOCIETY FOR ENDOCRINOLOGY ENDOCRINE EMERGENCY GUIDANCE: Emergency management of acute adrenal insufficiency (adrenal crisis) in adult patients
نویسنده
چکیده
Primary adrenal insufficiency is caused by loss of function of the adrenal gland itself, for example due to autoimmune-mediated destruction of adrenocortical tissue or surgical removal of the adrenal glands or due to inborn disruption of adrenal cortisol production in congenital adrenal hyperplasia. Secondary adrenal insufficiency is caused if the regulation of adrenal cortisol production by the pituitary is compromised, this can be the consequence of tumours in the hypothalamic–pituitary area. However, pituitary regulation of cortisol production is also switched off in patients who receive chronic exogenous glucocorticoid treatment with doses ≥5 mg prednisolone equivalent for more than 4 weeks. This may also be caused by long-lasting glucocorticoid injections into joints or chronic application of glucocorticoid cream or inhalers. In primary adrenal insufficiency cortisol deficiency is aggravated by a lack of adrenal aldosterone production, a hormone important for blood pressure and electrolyte regulation. This puts primary adrenal insufficiency patients at a somewhat higher risk of adrenal crisis.

Endocrine emergencies are potentially life-threatening clinical problems and are compounded by a lack of recognition leading to delays in therapy. Every endocrinologist is aware of patients attending their outpatient clinic, with tales of suboptimal care in a nonendocrine clinical environment because of a failure to understand their chronic condition and its possible complications. This is part...

Endocrine emergencies constitute only a small percentage of the emergency workload of general doctors, comprising about 1.5% of all hospital admission in England in 2004-5. Most of these are diabetes related with the remaining conditions totalling a few hundred cases at most. Hence any individual doctor might not have sufficient exposure to be confident in their management. This review discusse...

Adrenal insufficiency, primarily presenting as an adrenal crisis, is a life-threatening emergency and requires prompt therapeutic management including fluid resuscitation and stress dose hydrocortisone administration. Primary adrenal insufficiency is most frequently caused by autoimmune adrenalitis, and hypothalamic-pituitary tumors represent the most frequent cause of secondary adrenal insuffi...

Adrenal crisis is a life-threatening emergency contributing to the excess mortality of patients with adrenal insufficiency. Studies in patients on chronic replacement therapy for adrenal insufficiency have revealed an incidence of 5-10 adrenal crises/100 patient years and suggested a mortality rate from adrenal crisis of 0.5/100 patient years. Patients with adrenal crisis typically present with...
